When you have finished completing the questionnaire, it is important that you press “send” in the lower-right corner.

 

Once you have done that, you will get this confirmation:

If you have not received this confirmation, we have unfortunately not received your answer. You will be sent a new link within 1-2 weeks, where you will be asked to fill out the questionnaire again. You do not need to re-sign the consent, as you have already signed this before.
